CASE 3

Severe crowding – young adult

This 30-year-old patient presented with significant crowding in the anterior region, which affected both the appearance of his smile and his ability to maintain proper oral hygiene. The tight tooth position made cleaning difficult, leading to frequent tartar accumulation in the front teeth area.

An additional challenge was the presence of a conical lateral incisor, smaller than normal, contributing to a visible asymmetry. This was an important factor to consider when planning the correct distribution of space and achieving a balanced final result. Despite these issues, all teeth were in good health.

A treatment with Invisalign was carried out on both arches to progressively align, level, and harmonize the anterior teeth while improving overall symmetry.

The main alignment phase lasted 12 months, corresponding to 52 aligners (changed weekly). This was followed by two refinement phases of 3 months each, with 14 aligners per phase (28 additional aligners), allowing precise control and optimization of the final tooth positions.

To complete the treatment, composite restorations were performed to correct shape discrepancies related to the lateral incisor, and a professional tooth whitening protocol was added to enhance the overall brightness of the smile.

This case highlights how even complex crowding situations can be managed with a conservative, step-by-step approach, combining orthodontics, restorative refinement, and whitening to achieve a bright and balanced result.
